It might be useful to tell us more about your platform.

Which processor are you using and which kernel are you running on it?

What size is your NAND and and what page size does it have?

What YAFFS options do you have selected?

Without this info any reply can only speculate as to your problem,
though Ross Younger's reply is most likely pointing in the right
direction.
